In this role you will:
- Complete Care Act assessments.
- Complete Mental Capacity assessments and best interest decisions.
- Complete safeguarding enquiries.
- Cover duty for Crisis response and the wider hospital services such as hospital at home.
- Work with other teams across the HDTA service but also the council as well as our health colleagues/services.
- Support our citizens to remain in their own homes.
