Main duties of the job
You will provide high-quality, person-centred care to patients while they await discharge, supporting their physical comfort, dignity and emotional wellbeing at what can be an anxious and important point in their hospital journey.
Your will be supporting patients with personal care needs, ensuring privacy and dignity are maintained at all times. You will assist patients with nutrition and hydration, encouraging intake and supporting those who may need help to eat and drink.
You will record routine clinical observations as required, promptly escalating any concerns or changes in a patient’s condition to the registered nurse. You will help maintain a safe environment by supporting infection prevention and control practices, falls prevention measures and safeguarding processes, particularly for vulnerable adults and patients living with dementia or cognitive impairment.
A key aspect of the role is supporting patient flow. You will actively assist in the “pull” model of Discharge Lounge working, supporting their safe transfer to the Discharge Lounge once discharge is confirmed, helping to avoid unnecessary delays.
You will work closely with registered nurses to ensure patients have everything they need prior to discharge, providing reassurance and clear communication to patients and carers, helping them understand next steps while escalating any concerns appropriately.

- Demonstrates a willingness & capability to undertake NVQ level 3

- Demonstrable experience in hospital setting working at NVQ level 2
