Support Worker (developmental post) – Essex Partnership University NHS Foundation Trust (Visa Sponsorship).

An exciting opportunity to join the team and become part of the Mid Essex Dementia and Frailty Pathway this position is a developmental role in to a band 4 , and you will be required to complete care certificate and have the relevant maths and English within 12 months to move onto the apprenticeship.

The Mid Dementia and Frailty Pathway provides a comprehensive assessment, diagnosis and treatment supporting patients of any age with dementia as well as older adults, who have functional mental health needs with an associated age-related frailty. You will work as part of the multi-disciplinary team supporting patients with SMI / a secondary care mental health need/ dementia and frailty.

Main duties of the job

The post holder will work as part of the multidisciplinary team.

The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing

To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met.

The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests.

The post holder will arrange physical health assessments with the service user both at home, in clinics, and at GP surgeries; taking a proactive approach to engaging patients with SMI in the importance of annual physical health reviews.

To provide information on health promotion including healthy eating, sport and exercise and smoking cessation

To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring the information is shared with patients and GPs.

To promote positive mental and physical health to patients, carers, families and staff.

Person Specification

Education / Qualifications

Essential

  • Associate Practitioner Qualification / NVQ Level 3 or willing to work towards qualification.
  • Training in phlebotomy / ECG or willingness to undertake training
  • Care Certificate (or complete this within the first 12 weeks in post)
  • Experience of working with people with mental health needs in a paid or voluntary capacity

Desirable

  • GCSE pass in Maths and English
  • Experience of working in older adult mental health and dementia.

Skills/ Experience

Essential

  • Literate in IT/Computer Skills
  • Good oral communication skills based on fluency on the English Language
  • understand and communicate confidential client information of a sensitive and often complex nature, including discussing care with family members within boundaries of confidentiality
  • Able to make good use of clinical supervision in a group and/or individual format
  • Liaise with other teams and services including external agencies as required for the wellbeing of patients.
  • Able to deal with a range of clients presenting with mental health difficulties, their carers and the clinical team.
  • Able to work flexibly and adapt to each person as appropriate to their needs in a variety of settings

Desirable

  • Able to work in a variety of community. Settings which may include GP Surgeries, Community Clinics and patient’s homes
  • experience of working with System one or PARIS

Knowledge

Essential

  • Experience of working with people with mental health needs in a paid or voluntary capacity
  • Experience working as part of a team
  • Experience of being supervised

Desirable

  • Experience of reviewing, recording and communicating information

Personal Qualities

Essential

  • Current valid driving licence
  • Access to a car, and have the ability to travel across sites efficiently and across Trust boundaries to attend meetings, etc.
  • Shares the Trust’s Beliefs and models this in their attitude and behaviour.
