As a Social Worker, you will work as a member of the multi-disciplinary team, providing high quality social care services to adults of all ages experiencing mental ill-health. The core functions of the role will be to provide assessment, care planning and coordination of care from a social perspective to adults accessing the service. You will delivery recovery-focused care and support to service users and their carers that is focused on the principles of choice, control, and independence, as well as ensuring safety. You will act as a champion of social care and safeguarding in your locality.
You will hold a caseload which may comprise of adults experiencing a wide range of mental health and social care needs. You will ensure that you maintain constant reflection and development of your practice and will ensure your continuing professional development in line with the standards set out by the professional regulator, Social Work England
You will be Based on Beech ward . Beech Ward is a 15-bed low-secure unit for men with mental illnesses and a history of offending behaviour who are at significant risk of causing harm to others. It offers specialist psychological, psychiatric and behavioural therapies and complex specialist rehabilitation to help patients prepare for their return to life in society.

Roles & Responsibilities
- Be a champion of social care, including safeguarding, with an understanding of the relevant legislation and statutory guidance.
- Be an effective member of the multi-disciplinary team, providing high quality social care services to service-users and carers.
- Provide assessment, care planning and coordination of care from a social care perspective.
- Deliver recovery-focused care and support to service-users and their carers that is focused on the principles of choice, control, and independence, as well as ensuring safety.
- Comply with both child and adult safeguarding practice in line with current policy, including following up safeguarding referrals and child visit requests.
- Liaise with families and carers, and support nursing staff in arranging visits and developing good relationships with families and carers.
- Ensure the involvement of service-users and their carers so that the Trust can learn from their experiences to make service improvements.
- Liaise and negotiate with other agencies including the Ministry of Justice, Police, Children’s Services, Carers’ Networks, and other key stakeholders.
- Ensure appropriate use of the Mental Capacity Act, and support colleagues to apply this legislation.
- Ensure records of all work undertaken are accurate and up to date.
- Actively champion equality and diversity and challenge discrimination in all its forms.
Eligibility Requirements
- An appropriate professional social work qualification
- Education to degree level
- Registration with Social Work England
- Training in principles and application of outcomes focused assessments and personalization
- Relevant post-qualifying training for social workers
- Approved Mental Health professional/Best Interests Assessment Training.
- Training and experience in Safeguarding.
- Significant post qualification experience
- Knowledge of legislation, theory and policy in relation to relevant care group
- Experience of undertaking assessments, focused as personalisation, choice and control, and care planning
- Understanding and experience of personal budgets and direct payments
- Experience of multidisciplinary working and ability to work collaboratively
- Experience of partnership working with service users and carers, demonstrating dignity and respect
- Experience of work as Approved Mental
- Health Professional and/or Best Interests Assessor.
