Post-diagnostic Support Worker – South West London and St Georges Mental Health NHS Trust (Visa Sponsorship).

An exciting opportunity has arisen for a band 4 post-diagnostic support worker. The Wandsworth Memory Assessment Service is a well-established team, within a wider service, providing timely assessment, diagnosis and treatment for people with memory problems. The support worker will support the team to facilitate group sessions, care-plan directly with patients, liaise with family/carers, liaise with third sector organisations, and carry out audits. This is a fantastic chance to develop transferable skills and experience in cognitive disorders and older people’s mental health in a supportive multidisciplinary environment.

Salary: 33,094 – 36,195 £ / Year

Main duties of the job

This role requires someone with drive, energy and a commitment to high performance standards and the delivery of patient centred care. The post holder will ideally have experience working with older adults and have a passion for improving outcomes for older adults experiencing memory problems. You will need to have excellent communication skills, be pro-active and able to work autonomously. You will have experience facilitating groups or a wish to develop this skill, and experience working with family and carers. You will have excellent documentation skills and have an understanding of audits.

The role involves agile working, and the applicant will be expected to work from the team base as well as a variety of locations, depending on service need.

Person Specification

Training & Qualifications

Essential

  • Completion of Care Certificate and acquired competencies.
  • English and Mathematics basic qualifications (eg GCSE at Grade C or above or evidence of Level 2 in Functional skills for numeracy and literacy).

Desirable

  • Level 2/3 Health and Social care qualification / working towards qualification.

Experience & Knowledge

Essential

  • Clinical experience minimum 1 year and experience of engaging with service users / patients
  • Knowledge of treatment options

Desirable

  • Experience in different clinical care settings
  • Experience in supervision of staff

Skills & Abilities

Essential

  • Able to provide quality care that is responsive to service user’s needs, with supervision available but not always present, and acting on own initiative, within protocols, policies and care plans
  • Able to plan and co-ordinate and prioritise own and team work within a shift system.
  • Communications skills – receiving and conveying sensitive information, with staff and with service users and able to keep accurate records. Knowing when to escalate concerns and to whom Credible in a multi-professional environment
  • Ability to positively engage distressed individuals and de-escalate highly-aroused situations
  • Highly resilient and able to respond to difficult interpersonal situations, which may, at times, be traumatic / distressing.

Desirable

  • Training and experience in specialist skills such as ECG, Vital Signs, Phlebotomy
  • Experience in facilitating groups.
