Health Improvement Worker – Gedling Local Mental Health Team (Visa Sponsorship).

An exciting opportunity has arisen to join the Gedling LMHT. The LMHT provides specialised mental health assessments and evidence based treatment within the Gedling area. The service is committed to the principles of recovery; aiming for people to be empowered to be as independent as possible and wellness recovery action planning.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Main duties of the job

Gedling Local Mental Health Team are looking for an enthusiastic, dynamic and motivated Band 4 Health Improvement Worker to work across the Gedling area.

This would be working with the LMHT working with secondary Mental Healthcare Services; you would work with patients identified as having serious mental illness and cardio-metabolic risk factors.

People with a Serious Mental Illness die on average 15 – 20 years earlier than the general population often due to the under identification or poor management of their cardio-metabolic risk factors.

This role will involve working alongside mental health teams and general practices to proactively identify and help the patient to manage these risk factors and will involve coordination and communication between the different parts of the service that the patients are in contact with.

Qualifications

Essential criteria

  • To be trained to NVQ Level 3 or equivalent experience and have undertaken or be willing to undertake phlebotomy, venepuncture and courses related to physical health assessments

Desirable criteria

  • Completed Physical Healthcare training
  • Completed mental health awareness training

Training

Essential criteria

  • Evidence of training related to the role
  • Willingness to undertake training that will enhance service delivery

Experience

Essential criteria

  • Experience of working in Healthcare setting
  • Experience of building trust and a rapport with individuals
  • Ability to carry out interventions with service users within community settings, working closely with the multi-disciplinary team.
Desirable criteria
  • Experience of working in secondary/primary mental health care

Knowledge

Essential criteria
  • To have understanding and/or experience of the impact of serious mental health problems and the ability to use engagement skills to work with service users
  • To have an understanding of the role of physical wellbeing in mental health recovery

Skills

Essential criteria
  • Ability to work as part of a team
  • Excellent written, verbal and non-verbal communication skills
  • Ability to communicate with patients/clients and carers, using empathy and reassurance and where there are barriers to understanding e.g. learning disabilities or language
  • Diplomatic, caring & sympathetic
  • Ability to be flexible
  • Developed observations skills with the ability to report and record physical/mental health changes etc. observations of incidents or events and behaviours in an accurate and timely manner
  • Standard keyboard skills
  • Ability to lone work in a PCN but seek advice when required and escalate any physical/mental healthcare concerns
Desirable criteria
  • Ability to take blood using a vacutainer
  • Ability to use excel

  • Ability to work to meet the needs of the service

  • Breakaway
  • Manual Handling which may include manoeuvring of equipment /wheelchairs/trolleys/beds etc as appropriate
  • Frequent travelling around service area
