Main duties of the job

  • To be a core member of a multi-disciplinary Baywide Health, Wellbeing and Specialist Services Team and contribute to the effective assessment and discharge planning with working age adults in the area of Torbay and for Torbay residents in out of area hospitals in the region
  • To undertake holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust policies and guidelines
  • To manage the transitions inbox and referral process
  • To have clear responsibilities for the safeguarding of adults and children and for operating in a manner consistent with Human Rights legislation
  • To be compliant with and practice utilising social care legislation including Care Act 2014 and Mental Capacity Act 2005.

Person Specification

Qualifications and training

Essential

  • GCSE grade C or above
  • To have an NVQ level 4 in health & Social Care/or on appointment to commence the qualification within the HSCC role
  • Evidence of consistent pattern of learning from education, training and experience
  • Understanding of basic human needs – physical, emotional, health and social

Desirable

  • NVQ/CQF in social/ healthcare

Knowledge and experience

Essential

  • Understanding of basic human needs, physical, emotional and social
  • Ability to communicate effectively with clients, carers, specialist
  • Effective working as an individual but also as part of a wider multi-disciplinary interteam
  • Proven interpersonal skills
  • Ability to use time effectively
  • Effective recording skills
  • Commitment to multi-disciplinary working
  • Willingness to undertake training to develop within the role
  • Ability to prioritise competing demands
  • Ability to use IT based care management systems
  • Ability to work on own initiative whilst making positive use of supervision and training
  • Awareness of Care Act 2014 legislation
  • Assessment & Care Management Process, policies and standards
  • Effective keyboard/IT skills, which include accurate recording skills
  • Ability to undertake holistic assessments of persons need, arranging and reviewing services
  • Effective in using multiple data base computer systems
