We are seeking an enthusiastic individual with knowledge and experience of the care sector to join our team as a Assessment Co-Ordinator. In this pivotal role, you will oversee the assessment processes for new clients and manage referrals, ensuring that our service delivery aligns with our commitment to quality care.

Roles & Responsibilities

  • To assess new client care needs which involves face to face assessment meetings with the client’s and their families to produce the relevant documentation and person-centred care plan for our carers to follow
  • To carry out regular reviews of current clients, ensuring all client documentation is maintained and up to date with amendments made when there is any care support change
  • To liaise with KCC care managers, service users and/or their families and any other relevant professional bodies to ensure that the best interests of our service users are always maintained- this includes applying referrals for the relevant professional body required
  • Maintaining client records on our current computer system- including in-put of new client details
  • Completing care staff supervisions which includes staff spot checks and mandatory moving and handling training for all staff
  • General office/admin duties as required- ensuring all client files are correct and updated
  • To provide cover in the community if alternative arrangements/ cover cannot be provided
  • To cover the on-call service on a regular pattern in conjunction with other on call staff members

Eligibility Requirements

  • Hands on experience and knowledge of the care sector is essential
  • To be computer literate, ideally with a good knowledge of Microsoft products
  • Knowledge of Care Line Live an advantage but not essential
  • Must be a car driver with the use of a car
  • Must be able to work on weekends when rostered for our on-call service

The ideal candidate will need to be methodical with good organisational and communication skills. This role requires a team player as you will work closely with your colleagues within the office and those out in the field.

Benefits

  • Office position with involvement of care work and share of on-call (required).
  • Office hours – Monday to Friday 8am – 4pm
  • Salary £25,000 P/A increasing to £27,000 P/A after successful completion of probation period
  • Additional payments for on-call duties, approximately £3000 P/A*
  • Additional payments for any care support undertaken outside of office hours.
  • 28 days holiday increasing to 32 days after 12 months

  • Depending on your own qualifications, you will undertake Moving & Handing train the trainer, first aid train the trainer and medication competency qualifications paid by the company.
