Main Duties of the job
The successful candidate would offer support to Carers across the 4 areas of the black country Providing a time-based service. The service would offer carers:
- Hold a caseload
- A Carers assessment / resultant care plan
- Psychoeducation
- family consultation & conflict resolution
- Facilitate Trust wide Carers support groups/ forums
- Promote & deliver Triangle of care
- Advocacy
Person Specification
E = Essential criterion for the role D = Desirable criterion for the role.
Essential
- NVQ/QCF Level 3 in Health or Social Care
- Experience in mental health, social care, or Voluntary, Community, Faith and Social Enterprise (VCSFE) settings supporting carers or service users
- Experience in assessment, care/support planning, and review
- Understanding of the impact of mental illness on carers and relevant legislation (Care Act, Mental Capacity Act (MCA), Mental Health Act (MHA).
- Awareness of consent and information sharing, safeguarding, and risk management.
- Ability to manage a caseload, set boundaries, and maintain accurate EPR documentation.
- Ability to listen and communicate carers concerns
- To be honest and trustworthy
- Treat others with respect
- Ability to advocate on behalf of the carers at meetings, ward rounds and outpatient clinics
- Able to travel across the borough at short notice
- Empathetic, respectful, inclusive, and committed to co-production.
- Flexible and proactive approach to workload and working hours.
- Ability to travel across the Black Country (Essential).
Desirable
- Commitment to complete Level 4
- Relevant training in carer support, peer facilitation, or brief interventions
- Experience delivering group sessions or psychoeducation
- Experience supporting carers through MDTs, CPA, or discharge processes
- Understanding of the impact of mental illness on carers and relevant legislation (Care Act, Mental Capacity Act (MCA), Mental Health Act (MHA).
- Knowledge of strengths-based, trauma-informed, and culturally competent approaches
- Awareness of CORE20PLUS5 and Patient and Carer Race Equality Framework (PCREF) priorities.
- Good IT literacy (Outlook, Teams, databases).
- Basic conflict resolution and motivational interviewing skills (Desirable).
