Roles & Responsibilities:
- Deliver comprehensive patient care, including assisting with daily activities including bathing, toileting and medication administration.
- provide companionship, incouriment and support
- Transport patient to group meetings via our WAV
- Maintain detailed and accurate patient records in accordance with organisational standards.
- Collaborate with multidisciplinary teams to develop personalised care plans.
- Educate patients and their families about health conditions, treatments,
- Monitor patient progress and report any changes or concerns to supervising medical staff.
Eligibility Requirements:
- Proven experience of care for patients with dementia in the home setting
- Knowledge of anatomy, medication administration, and patient care procedures.
- Compassionate approach with excellent communication skills.
- Relevant certifications or licences in care support, also full driving licence.
- Ability to work independently and as part of a multidisciplinary team. This position is ideal for healthcare professionals committed to delivering exceptional patient-centred care within a supportive environment.
