We are now recruiting a Band 7 Specialist Community Palliative Care Social Worker for our community palliative care team in Camden and Islington. We are looking for someone who can show their resilience and motivation to work in this area, and also can demonstrate compassion, self-knowledge, confidence, generosity, and courage.

Main duties of the job

The post holder will work as part of the established Camden and Islington Community palliative care team across Camden and Islington boroughs. The successful candidate will provide specialist social work input for patients and families facing life limiting illness and bereavement, including the needs of the family before and after death, and for those living with serious illness, impending death loss and grief. The role involves independently assessing, planning, and managing a caseload of patients and their families, working collaboratively with other healthcare providers to ensure holistic and coordinated care. They will act as a specialist resource for consultation and teaching within the trust and outside.

Experience

Essential

  • Post Qualification Experience
  • Relevant experience including two or more of the following: Working with people who have palliative care needs Working with people who have cancer or a long term illness/disability Experience working in a health related setting Working with grief and loss Working with individuals and families Working with groups
  • Experience of working with people who are facing bereavement
  • Experience working as a member of an interdisciplinary team
  • A good understanding and knowledge of relevant social work, psychological and therapeutic interventions and models of grief work
  • Knowledge of current relevant legislation and government strategies including Safeguarding, MCA and DOLs
  • A good knowledge of palliative care and the role of social work
  • Experience of working with children and young people
  • Experience of delivering training and education

Qualifications

Essential

  • Registered qualification in Social Work
  • Registered with Social work England as a social worker

Desirable

  • Member of Association of Palliative Care social workers
  • Certificate/diploma/professional registration in counselling

Skills and Abilities

Essential

  • Ability to work with adults and children of all ages and backgrounds and demonstrate skills in assessment, therapeutic and group work
  • Excellent communication skills with an ability to communicate effectively, both in writing and verbally, with clients, carers, staff and external agencies
  • Ability to work as part of an interdisciplinary team as well as autonomously, managing and prioritizing day to day workload
  • Ability to work in a flexible manner
  • Ability to prioritize and manage a complex caseload
  • Ability to maintain accurate and up to date documentation with regard for confidentiality and agency policy and procedures
  • Self-resilience to be able to work in a challenging environment.
