We are looking for an enthusiastic and committed Community Support Worker to join our Intensive Support Team based at St Anne’s Centre, St Leonards on Sea support our Hastings & Rother Neighbourhood Teams.
The team works intensively with clients for up to eight weeks to reduce risk of admission and to support post discharge from mental health units. The team provides a liaison role and is in regular contact with inpatient services to facilitate discharge.

Main duties of the job
The main duties include
- Promote the independence of service users within the community setting.
- To provide regular and practical support to service users.
- To help service users gain access to resources to include benefits and welfare rights.
- To help service users access community supports such as day centres, clubs and day services.
- To ensure that the care plan is fully implemented by the service user in accordance with service specifications including liaising with and providing information to other agencies involved in the care of the service user.
- Monitor the quality of care delivered and service user and carer satisfaction with the service.
- Assist in reviews of service user in the community on a systematic basis so as to ensure the continued need and appropriateness of services provided by the Trust and maintain review records.
- Adhere to, and where appropriate, ensure the implementation of relevant legislation, Department and Trust policies and procedures.
- Maintain accurate and up-to-date records as required by the Trust.
- Participate in a formal programme of supervision.
Qualifications
Essential
- NVQ Level 3 in Care and equivalent skills acquired through experience
Knowledge/Experience
Essential
- Experience working within a mental health or social care setting
- Experience of working as part of a team
- Good communication skills.
